Forms

Forms lets you build custom forms to collect information from your members, or from the public. Use it for things like race result submissions, club surveys, event sign-ups, or consent forms.

Each form has a friendly public web address, and you choose whether respondents need to sign in or can submit anonymously.

Before you start

Admins need the Manage Forms permission to access Admin > Forms. See Permissions for permission setup.

Key concepts

Form A single form, made up of a title, an optional description, and one or more groups of questions.

Group A section within a form used to organise related questions together, for example "Personal Details" or "Race Results". Each group has its own title and optional description, and can be reordered by dragging it.

Field A single question within a group. Each field has a label, a type (see Field types), and can be marked as required. Fields can be reordered within a group, or dragged into a different group.

Response One respondent's answers to a form. Each submission creates a new response, viewable and exportable from the form's Responses page.

Published / Unpublished A form only accepts responses once it's published. Unpublished forms are not reachable at their public web address.

Login required Controls whether a respondent must be signed in to submit the form. Turn this off to run anonymous surveys.

Allow multiple responses Only applies when login is required. When off, a member who has already responded sees a read-only summary of their answer instead of a blank form. Anonymous forms never check for a previous response, since there's no way to identify the respondent.

Opens / Closes Optional start and end times. Outside this window, the form is shown as not yet open or closed to respondents, but it still exists and its settings can be edited.

How it works

  1. An admin creates a form, adds groups and questions, and configures its settings.
  2. The admin publishes the form.
  3. The admin shares the form's public web address with members or the public.
  4. Respondents fill in and submit the form.
  5. The admin reviews and exports responses from the form's Responses page.
